(JACKSONVILLE, Fla.) — A National Transportation Safety Board investigator classified the sinking of the cargo ship El Faro as "a colossal failure" in management Thursday while questioning an executive of shipowner TOTE Inc., WOKV News reported. "As you stated, the proof is in the pudding," Tom Roth-Roffy, NTSB investigator-in-charge, told TOTE Executive Vice President Peter Keller during the Coast Guard inquiry. Keller responded, "I think this tragic loss is all about an accident, and I look to this board, as well as the NTSB, to try to define what those elements may or may not have been. I, for one, with 51 years of experience in transportation, cannot come up with a rational answer. "
