Poor leadership, unsafe operation cited in Coast Guard boat probe

(ALAMEDA, Calif.) — Leadership shortcomings and poor safety habits may have led to the fatal accident involving a Coast Guard boat last year in San Diego Bay, the Union-Tribune newspaper of San Diego reported. During a criminal hearing, a Coast Guard commander said he witnessed unsafe practices, including crew ignoring lookout duty and using cell phones while underway. Three officers in the small-boat unit had been removed for inappropriate relationships for alcohol-related misbehavior.
