NTSB issues report on Axel Spirit collision with NY’s Ambrose Light

(WASHINGTON) — The National Transportation Safety Board has issued its report on a November 2007 collision involving the tanker Axel Spirit striking Ambrose Light in New York Harbor. The investigators said the probable cause was the ship master’s failure to use all bridge resources to identify his vessel’s course past Ambrose Light and failure to communicate his intensions to the bridge team.
